CDC Overview

Lessons learned from Hep A outbreak...what are best practices?  Worse outcomes, disproportionately affected.

Vaccine AmbassadorsVaccine Ambassadors:  community members who will do outreach, questions among these populations.  Focus is on COVID vaccine over the summer months.  Can move to other vaccines in fall–Flu, Hep A/B, HPV, Tetanus

NACHC Questions
  1.  Data collection??  The data could tell the story and measure impact.  We need to design it up front.  We have a data dictionary on COVID, vaccines, SDOH.  Otherwise will be qualitative. 
    1. CDC:  Evaluation measures should be collected.  CDC needs to brainstorm more about what elements those would be–data collection + evaluation
  2.  What is the best model of outreach?
    1. Model One: i.e. 20 HCH sites with their community partners would offer
      1. models
      2. data
      3.  workforce best practices
    2. Model Two:
      1. Work with different health centers to expand into that arena

Either model, it's building the infrastructure FOR the ambassadors.

CDC:  Ambassador would be based OUT of a community health center.  Would expand what that CHC or homeless is already doing.    Model One would be the first choice.

Amy PF:  important to enter all data into the IIS at point of care...these are mobile pops and are not tied to just one place...

Other models could include public health entities

Who are these Ambassadors?  CHWs, social services, enrollment ?  Does this align with Bidens funding for CHWs? Training?

CDC: 

Ambassador = CHW....at least a trusted voice (may have been homeless or SUD, but not required).  Health knowledge or open to training.  

Training?  Nat'l HCH  or NACHC could spearhead this.  

  • Ted is current running two Learning Communities:  working with CHWs on workflow on vaccine hesitancy..ending in June.  Can share lessons learned 
Next Steps
  • Kathy reached out to Bobby Watts (CEO of Health Care for the Homeless). who will reach out to his Senior Leadership team.  He is on board to support. 
  • Set up a regular meeting time–between 8am and 5pm
  • NACHC will write a workplan with budget within the next 30 days
