Steps to Launch Health Center and HCCN/PCA Meetings · NACHC review and feedback for: o Draft SOWs for health centers and networks and o Baseline data questionnaire topics · Kick-off meeting logistics for beginning of November: date, time, communication, and agenda · Update on outreach for informal discussions | Rocio Nicole Sarah | 20 mins | Participants · Sarah confirmed the number of 7 PCAs and networks and confirmed there have been no new additions since the last meeting: Alabama PCA (new), Alaska PCA, Alliance Chicago/Health Choice, OCHIN, Health Efficient (new), and the Dakotas (new). · Sarah confirmed the number of 6 health centers: Colorado Coalition for the Homeless, El Rio (new, returning), Holyoke, Mariposa, Primary Plus, and Winding Waters. · There will be 13 total participants, but Sarah noted that there might be more joining. SOW for Health Centers, PCAs, and HCCNs · Nicole noted the SOWs are nearly completed for NACHC to review, following CGH internal coordination. She reiterated, based on past meetings with NACHC, it will be designed to be broad across sprints so that they do not have to be updated after participants select a focus area. The SOW will state that a baseline questionnaire will group them into one of the four focus areas. Based on that determination, then the concrete deliverables and activities will be shaped. Sarah agreed with this approach. · Nicole asked to confirm whether personas should be included as one of the examples participants might work through. Sarah said there may need to be a persona refresh to remind participants that this project is about patients and families, not just data. She added that the networks in the COVID team project with the NACHC informatics team are doing personas again, and they may be able to use the same ones. Sarah reiterated that NACHC wants to stay true to the human design aspect. · Partnership activities: Sarah and Jennie clarified this would include streamlining ISS connections, harmonizing clinical and data workflows, CDS portals, as well as patient engagement strategies, care team extenders such as pharmacists, clinical pharmacist, community health workers, and dental. This focus area would identify and leverage existing partnerships. Meeting Logistics · Rocio and Sarah discussed dates to kick-off meetings with health centers, PCAs, and networks. · Timeslots for a health center kick-off during the first two weeks in November were proposed, and Sarah will verify the desired date with Kathy. · Sarah sent the currently scheduled meeting appointments for three of the networks, for a 20-minute discussion about the adult immunization work: Alliance Chicago/HCN, OCHIN, and Health Efficient. · Sarah said that the kick-off calls can be conducted without the SOWs in place. Sarah and Nicole agreed to continue discussion to determine if the health IT survey should be completed before the kick-offs or during the kick-offs, as well as shape the agenda for kick-off calls, including welcome of new members, introductions, overview of Sprint 1 and Sprint 2, and the direction to focus networks and PCAs together and separately from the health centers. · Sarah will email the health centers to introduce the CGH team, with draft text provided by CGH. |
