Proposed Activities and Scope – Phase I (as of 6/14/21)
The National Association of Community Health Centers (NACHC) has an emerging opportunity to harmonize clinical, data, and measure workflows to improve adult immunization rates among community health centers through a learning community. NACHC has a long and successful history of providing training and technical assistance to community health centers through learning collaboratives, allowing health center peers to come together to study and apply operations, leadership, and quality improvement methodology to focused topic areas. However, there is an increasing need for coaching and support in health IT configuration and workflow. As such, NACHC wishes to collaborate with Clinovations Government + Health (Clinovations GovHealth) to co-design an adult immunization learning community based on lessons learned from past and present vaccination-focused learning collaboratives.
To inform the design of the adult immunization learning community, this initial phase of work will focus on the following activities:
- Coordinate with NACHC: The Clinovations GovHealth team will attend weekly planning/coordination meetings with NACHC, as requested. These meetings will provide a venue to discuss project progress, roadblocks, and/or findings to date.
- Conduct Environmental Scan and Interviews: The Clinovations GovHealth team will review existing project content (across 10-12 projects) to understand goals and objectives of related NACHC initiatives to date. NACHC will provide project content via Confluence access and NACHC will orient Clinovations GovHealth team members to the location of relevant project artifacts.
The Clinovations GovHealth team will provide schedule availability for up to 10 interviews with project teams. NACHC will coordinate and schedule all interviews.
The Clinovations GovHealth team will prepare a draft interview guide/questions for NACHC to review and approve in advance of the first interview. The Clinovations GovHealth team will conduct and document the interviews using an agreed upon structure/format.
- Synthesize Results and Design the Collaborative: The Clinovations GovHealth team will synthesize interview and environmental scan findings and compile lessons learned and recommendations. The findings will be summarized in a three (3) to five (5) page brief which will be used to inform the design of the Adult Immunization Learning Collaborative.
The Clinovations GovHealth team, in collaboration with NACHC, will support development of a design plan and timeline, which will also inform scope and budget for subsequent phases of the project.
