Steps to Launch Health Center and HCCN/PCA Meetings

·          Pre-planning and coordination with NACHC: baseline data questionnaire and informal discussions

·          Sarah informed CGH the total number of participants is now 18. There will be 10 networks and there are 8 health centers.

·          Sarah noted Alliance Chicago and Health Choice Network  should not be counted separately.

·          NACHC is currently trying to get the El Rio health center onboarded; they participated a few years ago, but not last year. El Rio said they might not be able to participate until January.

·          Sarah said she is not sure NACHC will get all 8 health centers; they might end up with 6.

·          Sarah would like to see a kick-off by the beginning of November. She would prefer to have the SOWs completed and noted it would be ok to do a meet-and-greet with the participants without an SOW, but would like to have it on the way or to give to the participants shortly after the meet-and-greet.

·          Nicole proposed sending the data documentation/health IT ecosystem questions to the participants and/or having an initial conversation with them ahead of a formal kick-off. Sarah agreed and noted those conversations could be held during some of the already-scheduled meetings NACHC has with them. Nicole asked Sarah if she and Pedro could vet the questions first ahead of any conversations with the health centers. Sarah said she was going to sync up with Pedro this afternoon.

·          Nicole proposed crafting high-level SOWs to expedite getting them to the health centers. Sarah agreed but noted NACHC’s financing is going through a shift; the SOW line items now have allotted dollar amounts, so Sarah would like a combination of high-level and targeted since the SOW can only be done once. Deliverable dates can be broadened and flexible (i.e. deliverables don’t necessarily have to occur in order).

·          SOW language may differ between networks and health centers if, for example, the products of a deliverable will be different for each.

·          HCCNs and PCAs are asked by NACHC to pick 5 clinics, but they usually pick 2 – 3, historically.

·          Anita suggested the SOW language should specify 5 clinics; noting clinics may push back, but 5 should be stated

·          Participants are getting $150,000 for their involvement in this project.

·          Sarah said for the conversations with the participants, she and Pedro will discuss using time the networks have and adding to it or setting aside time on Tuesdays and Thursdays but will circle back to CGH after connecting with Pedro. She anticipated sending CGH her and Pedro’s availability by 10/15/21.

Informal Discussions

·          Nicole researched NACHC’s recommendations to speak with Dr. Bridges and Dr. Tan and would like to reach out to them. Sarah agreed, said for Nicole to use Sarah’s name in the outreach, and noted she would not need to be part of that conversation.
