HIT-Enabled High Quality Post-Partum Care

1) Identifying patients who are pregnant, pregnancy risk factors requiring follow up and scheduling post-partum visit

• Pregnancy episode and data stream of delivery
• Risk factors including: HTN and cardiovascular disease, GDM, mental health, substance abuse
• Operational considerations of post-partum care-- coordinating with neonatal visits, linking records and engaging pediatrics, coordinating with hospital care for follow up

2) Ensuring patients have timely post-partum care

• Dashboard for post-partum visits
• Flag for missed appointments

3) Appropriate care activities in the post-partum period

• Contraception
• Complications of pregnancy
• BP management
• GDM screening
• Post-partum depression screening and tx
