Goal Statement

Goal: To improve quality postpartum care services for health center patients

Objectives:

  1. Identify pregnancy episodes and patients who should receive specific postpartum care services

  2. Increase the proportion of all postpartum patients who receive an initial postpartum visit with their obstetrician–gynecologists or primary care providers based on current or existing guidance and recommendations

  3. Increase the proportion of postpartum patients who receive quality postpartum care services

    • Cardiovascular care (hypertensive disorders of pregnancy)

    • Diabetes screening for women with GDM-affected pregnancies

    • Postpartum depression and anxiety screening, referral, and treatment

    • Contraceptive counseling and provision

    • Substance use disorder screening, referral, and treatment

    • Other evidence-based recommendations for postpartum care services

AllianceChicago (Alliance). Alliance is a CHC-controlled health network comprised of 68 national CHCs serving more than 4 million patients annually. All member clinics are Public Health Service 330-funded FQHCs with federal mandates to provide care to the medically underserved. AllianceChicago CHCs share common EHR platforms which are centrally managed to promote quality improvement and research initiatives. 

Aliados Health. Aliados Health is a network consisting of 18 CHC members across 6 counties in Northern California. Member CHCs provide care to nearly 300,000 predominately low-income patients; 55% percent of whom are Latino and 45% are best served in a language other than English.

Health Choice Network (HCN). HCN is a CHC-controlled network that consists of 30 member CHCs across 9 states providing care to more than 2 million patients. HCN has a shared EHR system and robust data and administrative infrastructure to support research initiatives. They have expertise in stakeholder engagement and are actively committed to research endeavors at member CHCs. They are a member of ADVANCE, one of 9 PCORnet research networks and longstanding research partners with AllianceChicago.
