The Year 4 Measures


A3. Establish monthly data collection for data in these five areas:
• Pregnancy status
• Identifying high-risk pregnancies
• Delivery data exchange
• Post-Partum Visit
• Comprehensive post-partum evidence-based Care

A4. Integrate learning from previous year to design Health Information Technology (HIT)-enabled quality improvement strategies implement an iterative process of using data to improve the interventions to improve results for comprehensive post-partum care including
• Cardiovascular (blood pressure, cardiomyopathy)
• Diabetes/GDM
• Bleeding
• Post-partum depression/
