  1. I would like to see across the board increase collaboration with patients in the delivery of their care. 
  2. For Alliance, the build we propose will not only help track these patients but give more opportunity to discuss this with these patients.

  • There is an established, formal process for quality assurance.
  • The data are shared with centers to check their impressions of validity.  They work with the CHCs to make sure that the codes work for them.

  • Providing baseline data to the CHCs
  • post partum depression screening is of interest to CHCs


Data






  • Who are the pregnant patients? How many had successful deliveries?
    • What is the contraception distribution in this group
  • How many pregnant patients received Follow-up/ Post-partum care
    • What is the contraception distribution in this group? Is it different from the overall pregnancies?
  • What percentage of pregnancies were diagnosed with Gestational diabetes?
    • What percentage of this group received appropriate Diabetes follow up?




  • OCHIN has a pregnancy outcome field

  • Need to see if we can filter out patients who got same test twice
  • Need to look closer at gaps, and how these data align with the WHC UCSF data.
  • Contraceptive method among derived among all suspected pregnant
  • At the bottom, it's all who indicated post partum visit using post partum code.
  • Time period - if they received a contraceptive test, an estimated due date. The one below is specific to post partum visit.

All this work and these tasks are a heavy lift, and are preparation for the Quality Improvement work that can begin in March.
