1) How many patients are tested: 

• tests positive
• tests intermediate and negative
• by demographics

2) How many people have COVID-19 diagnosis?

• Confirmed positive test
• Per pt report
• Clinical diagnosis
• Suspected diagnosis without confirmation or refution

3) Flu vaccine provision: UDS patients, all patients

• Reported flu vaccination status
• Refused
• Confirmed via CPT or CVX
• prepare for COVID vaccine provision

4) SDOH needs of COVID-19 patients

• social services provided
• SDOH as a predictor of risk

5) Outcomes COVID-19

• Days to recovery
• Number of visits with COVID-19 dx
• Hospitalizations and deaths?

6) COVID-19/ILI Symptoms

• testing of these patients?
• Follow up diagnoses-- flu, COVID, URI, etc

7) Risk stratification
