Which patients are included? How do we know which are UDS and which are limited service?

How many and which health centers are included? what is the label for a health center (ID, name, etc)

Demographics: Do you currently use the categories in the Data Dictionary? If not, can they be normalized to subset aligned with UDS or other national definition?

SDOH: do you support PRAPARE? Do you support other tools? If so what other tools? Do you have other SDOH data mapped to PRAPARE or some other standard?

Are only tests performed at the health center included? Can you link to LOINC code for those done at the health center? If LOINC is not available what is?

Are only vaccines performed at the health center included? How are historical vaccines marked or is there no way to differentiate? Would a CPT code plus a vaccine be a sign of a health center vaccine? Does your data only have vaccine administration or vaccine product but not both?

Is there guidance for COVID 19 diagnosis codes in your health clinics? Do you perform any validation to look for patients who test positive to see if they have diagnosis codes or confirm cases otherwise?

How do you identify post-COVID syndrome patients (see value set in data dictionary for specific codes)? Have you used multiple methods to do this? Is there a local code or identifier for Post-COVID Syndromes? Do you have dashboards or measures of Post-COVID patients you use or provide at your organization?
