SDOH Closed Loop Pilot Proposal

Purpose

Development of closed loop protocol for Transportation and/or other SDOH (e.g. interpretation) to evaluate the effectiveness for improving patient care outcomes 

Major Products  

  1. Identify a population of focus.  Explore by age, chronic disease, risk factors, insurance coverage, and adult immunizations.  

  1. Closed loop referral workflow protocol that is developed in health center setting  

  1. Report including user stories, promising practices, and lessons learned for using closed-loop referral protocol 

  1. Closed loop resources for dissemination to other health centers nationally  

Deliverables

Deliverable 1: Communications  

Tasks/Activities: Teams will engage in regular communications and participate in learning meetings with AAPCHO to share lessons learned, promising practices, project challenges and solutions, (e.g., workflow implementation, user stories, data documentation strategies) that will contribute to development of a closed loop transportation (and/or other SDOH) protocol. The meetings will also serve as a process for tracking practice efforts, collecting use cases, and seeking input on how the protocol should be revised for national dissemination. The meetings will be conducted via Zoom but may include travel to health center partner site during the development or evaluation phase.  

Products: Compiled recommendations from health centers and stakeholders on design of closed loop referral system for Transportation or other SDOH  

Timeline: February 28, 2024 

Deliverable 2: Implementation Protocol Design  

Tasks/Activities: Design and develop required materials for implementation including data collection workflow process, data template, evaluation tracking measures as well as training and change management plans (e.g., assignment of key roles such as data collection champions (e.g. MAs, CHWs), provider champion, referral staff champion, IT champion) to maximize staff buy-in and implementation success.  

Products: Data collection implementation resources including workflow diagram(s), data template, data reporting/tracking templates, and screenshots  

Timeline: March 30, 2024 

Deliverable 3: Implementation Readiness Assessment  

Tasks/Activities: Develop health center readiness assessment that can be disseminated to other health centers to assess capacity for conducting SDOH closed-loop referrals. Test the readiness assessment with health center to confirm preparation for implementation and address gaps as needed.  

Products: Developed and tested Implementation Readiness Assessment Tool 

Timeline: April 30, 2024 

Deliverable 4: Implementation and Evaluation  

Tasks/Activities: Implement PDSA workflow with data collection and testing in health center setting. Pending additional funding for health center(s), activities may include: data collection, data reporting, evaluation, and sharing of results with health center(s) for validation and interpretation. At minimum, AAPCHO will collect and compile implementation perspectives from participating health center(s) and stakeholders.    

Products: Narrative Report that describes key findings, promising practices, and lessons learned as well as recommendations for next steps
