Adult Immunization
Why and What We Do
LEARNING COMMUNITY
The COVID-19 pandemic has caused significant disruption to the nation’s public health and healthcare infrastructure. In Year 4 of the Strengthening Public Health Systems and Services through National Partnerships to Improve and Protect the Nation's Health by strengthening adult immunization practices in health centers serving high risk vulnerable populations, the National Association of Community Health Centers, (NACHC) will apply lessons learned from COVID-19 to address Flu, Pneumococcal, and other adult immunizations.
YEAR 4 LEARNING COMMUNITY STRATEGY:
HUMAN CENTER DESIGN (HCD) LEARNING MODEL ELEMENTS:
COVID-19 SPECIFIC PROJECTS: MESSAGING MATTERS, STRIKE TEAM, VACCINE AMBASSADORS
The COVID-19 pandemic has caused significant disruption to the nation’s public health and healthcare infrastructure. Community health centers serve as the safety net to the highest-risk patients. The National Association of Community Health Centers (NACHC) proposes to engage local health centers as trusted brokers to build vaccine confidence in high-risk populations by empowering individuals and communities with accurate accessible information about COVID-19 and vaccines.

HEALTH CENTER INNOVATION GRANT PROGRAM
The COVID-19 pandemic has caused significant disruption to the nation’s public health and healthcare infrastructure. Community health centers serve as the safety net to the highest-risk patients and often serve as first responders to emergencies such as hurricanes, fires, and public health crises. The National Association of Community Health Centers (NACHC) proposes to engage local health centers as trusted brokers to build the evidence base of effective interventions to reduce disparities in adult vaccination rates amongst diverse populations (homeless, mobile, rural, refugees, behavioral health, LGBTQ, farmworker, non-English speakers,
immigrants etc.).
