1. I am using Azara (or another population health platform) that does not allow me to query for tables that are formatted as they are in this document. Can I submit my data in another format? |

Please send an email to Jen Du Mond, Manager of Data Quality and Analytics (jdumond@nachc.com), with a note about the situation. In most cases, an alternate format will be acceptable, but we need to know what to expect. In some cases, other data partners might have had the same concern about the same platform and found a solution. |
2. I don't have access to some of the data fields. What should I do? |
The most essential data elements are in bold red font on the Definitions tab. It is our preference to get data for each of the tables and columns, but if the non-bold red columns are unavailable in your system, we will accept the data without them. |
3. If I can't submit MRN, but you need a patient identifier, what can I use instead? |
If your organization does not have an established policy for de-identifying data, we may be able to offer some suggestions. Common options include using a hashing algorithm to consistently encrypt the original value or keeping a separate Excel file that assigns sequential values based on random sorting. Please contact the data team if you have any questions or concerns about your approach. |
4. My data governance policy requires that I further de-identify the data (e.g. 3-digit zip code, date shifting). Is this acceptable? |
This will need to be resolved on a case-by-case basis. Some fields need to be very precise for our analysis to be accurate. In general, the Limited Data Use Agreement should allow you to include unmodified data but if you are concerned, we will work with you determine how you can adhere to your policies while providing the data that is necessary for the project. |
