SECTION 4: Successes, Challenges and Additional Information

21. If the program were to feature your organization in an upcoming webinar or program-wide communication, what new, major successes would you like to share?

If applicable, include the web addresses related to the new, major successes mentioned in your response.

We would like to share that we have administered various doses of the COVID Vaccines in NEVHC. We have data below of patients/community members by age/ race/ethnicity.

NEVHC won the VAXUP Challenge and were awarded funds to be able to implement our solution of motivational interviewing to address COVID 19 vaccine hesitancy in the pediatric population. Another success has been that we have implemented COVID Plus clinics to our current workflow. COVID Plus clinics are clinics that still offer COVID vaccines and while demand is low also offer blood pressure checks, BMI checks, schedule follow up appointments for diabetic patients, and schedule follow up appointments to patients that may be due for retinals screening, pap smears, & mammograms. Lastly, we also offer FIT Kits and education for patients to complete their colorectal cancer screening.

22. What new, major challenges would your organization like to discuss during future program check-ins?

· Learning more about how our patients/community members feel towards all immunizations is a challenge. Focus has been primarily on COVID Vaccines.

· Additional guidance on how to select a population or multiple populations to focus in our interventions.

23. What additional information would you like to share about any partnerships with local organizations, coalitions, community projects, vaccination providers, or health departments? List the names of new partners and their contributions to program activities when possible.

We have been able to learn more about the community and their needs/barriers. Pacoima beautiful has the capacity to complete door to door outreach and live calls. VCCC has contributed by interviewing community members.

24a. What would you like to share about equipping influential messengers?

Equipping providers to have open-ended discussion with patients to develop and maintain a trusting relationship. Equipping providers with motivational interviewing skills will reduce burnout.

24b. What would you like to share about vaccination opportunities and provider partnerships?

Based on patient feedback gathered during interviews, patients expressed barriers with access. NEVHC and partners will further promote their various vaccination opportunities to their patients and families.

· Proper education regarding vaccination and vaccine components

· Need to empower patients to ask their provider questions
