Vaccine Ambassadors - Reducing Racial and Ethnic Disparities in Adult Immunization Program

Vision, Mission, Objectives

Draft statement 5/14:
The COVID-19 pandemic has caused significant disruption to the nation's public health and healthcare infrastructure. Community health centers serve as the safety net to the highest-risk patients and often serve as first responders to emergencies such as hurricanes, fires and public health crises.  The National Association of Community Health Centers (NACHC) proposes to engage health centers and community partners who serve  individuals experiencing homelessness, substance use/abuse and human trafficking. As trusted brokers NACHC proposes to build vaccine confidence and reduce access barriers in these  high-risk populations through the use of Vaccine Ambassadors and Community Health Workers.  This Approach to Reach Vulnerable Populations and increase Vaccine Acceptance includes a package of mechanisms:

  • Partnerships  with key national, state and community stakeholders to support the work of Vaccine Ambassadors including the National Health Care for the Homeless Council, state and local health departments, and community organizations.
  • Engagement and use of Vaccine Ambassadors or extended care team such a peer to peer navigators and community health workers  to reach patients/communities impacted by homelessness, substance abuse and human trafficking.
  • Engagement of up to 25 organizations with national representation with a cross section of urban/rural, region of the country, race and ethnicity to engage and co design the work.
  • A learning community with expert faculty and participating organizations (and vaccine ambassadors) to harvest lessons learned and  share promising practices.
  • Use of a point of  care patient centric care plan to link patient needs with social services, medical care, electronic health record and IIS. Initial data points to consider
    • Vaccine Hesitancy
  • COVID prevention
  • COVID treatment
  • Housing insecurity/ lack of stable housing
  • Food insecurity
  • Lack of access to health care
  • Dental need
  • Medication access
  • Referral
  • Post-COVID syndrome
  • Substance abuse
  • Mental health treatment need
  • A communication strategy  to share lessons learned and promising practices.
