Dental director seeing increased tobacco users, so linking up with behavioral health to close the gap

Working on closing the loop between referral and follow-up.

• Meeting monthly with CHCs 

Outpatient health center may need to drop out.  Two other health centers (including one with tobacco financing from NFL)

--Daughters of Charity

Azara onboarding at 26 health centers.  Working on developing specialized tobacco reports.  Found that UDS measure measures SO MUCH  (one screening every 24 months).  

Using Azara to identify patients PER visit who are "due" for tobacco screening

Brandi will send notes from these meetings.

Reviewed summary scope of work/deliverables.  Met all Deliverable 1 requirements, and can now invoice.
