Good Morning,

The Louisiana Primary Care Association (LPCA) has completed deliverable #1 of the CDC/NACHC tobacco project, “Addressing Tobacco Cessation in Primary Care”.  We have been cleared to submit for $26,000 as payment for deliverable #1 completion.  Below is a lists of those deliverables and how they have been noted as complete in addition to supporting documentation if needed.

Deliverable 1: Project Implementation and Updates

Tasks/Activities

• Ensure participation of three health centers in testing change concept, on calls and submitting progress reports – word documents attached for monthly calls
• Participate in regular webinars with representatives from CareSouth Medical and Dental, Outpatient Medical Centers, Inc, and Priority Health Care health centers and NACHC – LPCA and its centers have participated in all calls with NACHC through March 2021.
• Provide expertise and support to health centers on tobacco cessation with attention to an interdisciplinary approach – LPCA along with all heatlh centers are participating in all learning collaborative webinars hosted by the Louisiana Tobacco Control Initiative (TCI) and the Louisiana Tobacco Free Living Program
• Assist centers to apply for the Million Hearts® Hospitals & Health Systems Recognition Program – this is ongoing but centers need more time to complete their projects
• Provide evidence of reaching out to state Tobacco Control program – documented notes and calls with NACHC assured LPCA’s project officer is aware of the partnership with a statewide tobacco control program – TCI.
• Submit mid and final progress reports to NACHC – see attached word documents
