Welcome to the Tobacco Cessation Project Space!

This is the home base for the Tobacco Cessation project in Confluence. This is the place where we can collaborate, plan for the project, and keep important documentation.




Goal Statement

Strengthening Public Health Systems and Services through National Partnerships to Improve and Protect the Nation’s Health by implementing an evidence-based quality improvement (QI) initiative to improve health outcomes by applying CDC Tobacco Cessation Change Package.

The evidence-based quality improvement work to improve tobacco cessation strategies that work in health centers in three areas:

  1. Select a population of focus
  2. Partner with state tobacco and Million Hearts programs
  3. Apply Human Centered Design and Implementation Science Strategies
  4. Identify one domain from the CDC Tobacco Cessation Change Package to test using iterative QI strategies.
  5. Share lessons learned through a virtual learning collaborative.


NACHC and contractor will apply Human Centered Design Framework tools and strategies to develop a rapid cycle intervention through peer learning, iterative data submission and QI strategies.  Results will be shared nationally through a communication strategy.
