Site was working on this project - using eisting OCHIN tools on pops that would need screening - they increased the screenings by 3,000 ppl. Provider meeting 2nd week of May. They specfically targeted ppl who had not had a screening - they called the patients - the new routine would add alerts and be more related to adding testeing to routine visits.

  • Cultural issue of automatic test ordering vs provider manually ordering?

    • We would like to learn more about how sites feel about automation and implementation of this kind of thing.

    • OCHIN might assess readiness with partner sites - providers tell us that they want less burden, more automation; how do people at the site feel about that? What is the general feeling about automating HIV testing - it’s a culture shift that CDC is looking for, normalizing HIV testing, and automating the order.

    • Julia indicates manual confirmation is still ok - but the ordering would be automatic.

    • NACHC is developing an app to help patients get information and create a plan for HIV prevention for themselves. The app will be available in the app store and google android. The app helps people assess their risk, and health centers can send us their material to input so that when someone says they are from that zip code, that HC’s info can come up. NACHC will issue news about when the app is available.

Areas that we discussed:

Michele - Make the post-partum visit earlier for high risk patients. What would we do with that information once we have it?  

On post partum report - put a column so we know that the patient is a high-risk patient, and we know when their next appointment is. - can make a checkmark that they were a high-risk patient for post partum report.  We would have to make some decision about post-partum care and if there is something we can do to better track when they are post partum.  

Issue is coordination - hospital should be notifying clinic... OCHIN does get messages, they go to the primary care physicians or does it go to primary ob/gyn.  The messages don't have obvious content so they are ignored.  They are automatic and someone has to weed through them. It is easy to do, but hospitals don't do it - they should be sending by direct secure message.  It's easy if there's continuity - if it is a hospitalist that delivers the baby you don't get the communication.  Elliott's comment - if we could get buy-in (and define the carrot and the stick)... 

This one could be dropped or made a part of the measure flow - Screening for HTN and proteinuria during pregnancy, because it is not in the post-natal period.

The question is: Do any of our sites have any mechanism where if they were high risk in pre-natal, they get flagged to follow up for post-partum.  Michele - we could ask sites if they feel this would be helpful?

Next step - ask partners - an email to partners asking them to consider "birth plan" care or a path to do the referral; there is a way to do it systematically.

No guideline how long after delivery they would dx dm after delivery - but it can occur up to a year after delivery.  Do we still want to consider only the 8 weeks?   Jayson may have correcfed it to 16 weeks.  We could just look at the year after the birth.



Post partum depression - we are seeing codes for major depression disorder but there ot a lot of codes for post-partum depression.- should we still rely on the post-partum depression code?

Yes - BPA for that PPD code instead of major depressive disorder code if it is during the post-partum period.

  • Still customizing reports, found in process with Southern Jersey that they had a lot of gaps in education as well as reporting (education related to documentation and reporting), trying to customize reports for COVID and pneumovax, trying to loop in reporting person there. Supporting them on education related to clinical decision reports, found gap in staff training related to that and have shared internal OCHIN resources;

    • Meeting with them this week or next week to finalize date - some sites do not have connection to state IIS, OCHIN is working with them on that as well

    • Waiting for definitive feedback on final report, no new numbers because they have found more gaps

    • Some staff (community outreach for example) don't understand the basic triggers in EHR that help them figure out who is due for an immunization, etc. - some level of training happening, these resources are already available at OCHIN - what information would help head of vaccinations (Director of nursing, hasn't really run these reports herself before) understand vaccination data? OCHIN is working on this providing resources and trainings

  • Last learning community session coming up in June - how have HCs used the data that OCHIN/NACHC has been able to give them? 

  • Timeline on data? Seren will check in with data person; shouldn't be longer than end of this week

  • Next LC Learning Session focused on SDOH - Interest in being spotlight partner for upcoming LC session? Aiming for 5-10 minute presentation (potential topics from CGH: on anything to help HCs document SDOH, guidance on how to capture, how to apply SDOH data);

    • OCHIN works regularly with HCs on SDOH but not necessarily related to adult immunizations, could speak on SDOH in general? May need more guidance on finding overlap with AIM; OCHIN has resources/coaches relating to SDOH - could present on support they offer to SDOH that aren't OCHIN Epic specific

    • CGH: Other HCCNs/PCAs probably face similar issues, general SDOH would be a great topic for a brainstorming issue, how to share that information with HCs - OCHIN 

  • Third party tools that OCHIN offers to HCs? 

    • Yes, ex. for telehealth, they have a list of different integrations they have

    • For AIM - not sure if there are any

  • Potentially discussing experiences with third party tools, where they are helpful, if they could be useful for adult immunization for outreach, tracking, etc. - is that a topic that might interest OCHIN? 

    • OCHIN interested, one of their HCs interested in using one for AIM but not sure - not sure if interested in discussion if focused on reporting tools since there are already tools for that within Epic, they have people they will ask to see if there is interest

Look at proposed measures: how much are we able to track depression screening?

to what extent are we able to id the high risk patients who have CVD, substance abuse, severe mental illness, cancer or active bleeding issues.  We'll be looking at those value sets retrospectively.

Is there a consistent way to label high risk patients?  In a generic way "these are high risk patients" or "these are high risk CVD patients; these are high risk substance abuse patients"

To what extent are we able to track things like post partum HTN or DM in the EMR?

  • what does the care team need?   Lots of interest in high risk patients to help the team id patients for follow up.  

  • To answer the question: whom should I be worried about who is not in my office?  Did they transfer care? That might be an outcome of pregnancy episode.

  • Mark will need time to work on this data pull and we will likely not be able to submit this until mid-January 2022. Fine to submit in mid-January.

PROVIDE MISSING DATA FROM JANUARY 2020-JUNE 2021 AS IDENTIFIED BY NACHC DATA TEAM USING THE DATA DICTIONARY AND ANY UPDATED VERSIONS PROVIDED BY NACHC STAFF.

  • I don’t understand what the missing data is referring to.

  • Andrea's guess: this a cut and paste error - originally it was was for partners that participated in Year 1 and had missing data.

Provide monthly patient-level de-identified data for September – January 20212 using the Data Dictionary or an updated version as requested by NACHC staff. Assuming this is meant to say September 2021 - January 2022, and if so, we can submit June 2021-January 2022 together once we understand the data request better. If you’d prefer they be split then that’s okay too.

Review of SOW

Scope of Work re: number of health centers, challenges



  1. Patient level data is required, so patient-level DUAs are required.

  2. How many CHCs?

    1. 10 are on the Azara... there is a DUA for patient-level data that already have protections for de-identification. 10 is fine.

    2. Do DUAs with all of them and work with 4 for data improvement projects.  Improvement projects are not required on this project at this point.  

    3. We don't have additional years of funding at this point (because when this was funded CDC did not know how long this would need to last), so QI was not a part of the original one-year plan. 

    4. One strategy is to begin with the 2020 data for the CHCs you have and have DUAs from; you can continue to talk with additional partners about bringing them on board.

    5. You can consider all of them part of the project even if they do not contribute individual-level data - they may have insights and/or aggregate data that could help us - it's not a matter of "in" vs "out."  More are welcomed but not required.

    6. No zip codes, no birthdates - just age.  The purpose is surveillance, not individual-level analysis.  The only reason to have individual-level data is to see if there are patterns.

    7. There are no CHCs with DUAs at this point; the Alaska PCA team will be meeting with the CHCs on Jan 20, 2021 

      1. Aggregate data now

      2. Aggregate data now and individual data later (YES) when DUAs are in place.

      3. Patient-level data later





Status of work with Azara



  1. Alaska, Louisiana, and Azara met Jan 11, 2021.  Azara will mine the reports and send them to NACHC.

  2. Can get all in data dictionary - just a few things they will not provide

    1. MRN, but will have a patient identifier so NACHC can look for individual-level patterns





Data Status and Discussion



  1. The number of tests is very high, number of positive tests is high... looks like more tests than patients

  2. Sometimes the encounter is not a UDS-qualified encounter - so impacts the denominator... 

  3. Can we tell how many of their CHC patients (not walk in non-patients) were tested

  4. @Julia Skapik (Deactivated) suggests giving NACHC updated aggregate data on the 10 that are on Azara ... @Pedro B Carneiro (Deactivated) agreed that NACHC had received it.

  5. Azara needs to create the reports so Jan 15, 2021  deadline is not likely

  1. Indian Health Service (IHS) beneficiaries are getting immunized through their IHS centers, not through CHCs

    1. Age 50+ using IHS vaccine allocations.

    2. Have done care providers

    3. Anchorage 65+  they vaccinated 800 ppl on Jan 10, 2021 

  2. Non-IHS Clinics

    1. Bethel Family Clinic, non-tribal, did immunizations.

    2. Not all non-tribal got vaccines to give out.

    3. Clinic where @Former user (Deleted) went had no vaccines.

    4. Others may be doing 400 or so.
