Pre-Work | Quality manager Joyvell uses i2i population health software to gather data for Crinela Todea patients (vaccination status for COVID and pneumococcal). Mariella RN performs chart reviews for missed vaccination opportunities (random chart reviews). Current plan is to increase vaccination rates in one provider at one site, do PDSA, then add second provider at another site, PDSA one week after. Will consider targeting special populations for pneumococcal vaccine--current idea is for diabetic patients aged 19-64. |
