RE CMQCC


  • ID patient group that are higher risk for HTN and CVD


AC has a Merck funded project - Ta Yun has developed an algorithm to ID high risk patients.

  • Could this be turned into value sets
  • Doing implementation
  • Will be doing a one-pager to describe



Could find funding to augment - to build a playbook based on OCHIN and AC an DCMQCC.



UCSF - any concerns?


  1. Timeline - Can we really start on October 1?  What would be a timeframe that is more realistic?  End of recruitment December 1 - all partners are on board for pre-data collection starts January 1st.
    1. OCHIN reasonable - has 3 solid, Christine will speak wat their member work group meeting.  1 maybe - Trucare.  A couple of maybes said no.  A couple said maybe in 2022. 
    2. SINC questions"
      1. Must use wording? Must record the intent - most say: we really like what we have.  Get Julia to say that again
      2. Did the patient get asked about intent?
      3. What was their response?
      4. Resistance? Changes to workflow? Not feasible.  Even asking ppl to do the PCCC - it is not a huge burden, but ppl are saying they cannot add that.  Working on convincing ppl. 
      5. Any ideas on how to automate?  Use care message light? To develop a survey.
  2. AllianceChicago - ICAN? Project being implemented - on contraception - pretty much the same project except SINC question. 
    1. Could work out agreements to do both ICAN - 2 sites. 
      1. One has data that does not come into infrastructure at AC.  Does it need to be a partners whose provider data in the warehouse?  Yes we could work with that
    2. 2 certain, 2 potential, 2 strategizing…maybe 3 by January with data.
