Questions and Answers

  • We have 11 different NextGen users in the state and that all of them are running into the same barrier and its becoming a burden on reporting to payers as well as state/national partners. The PRAPARE Assessment Tool in NextGen (version 5.9.3 and later) is great, but still missing some key information necessary for documentation. 
    • The Material Security questions (food, phone, utilities, etc.) still needs an option for “Choose not to respond” for each of the 6 elements. There will be times a patient doesn’t want to respond about child care for instance, but they will answer all other elements. At this point, to document child care it would have to just be left blank as there isn’t a “choose not to respond” option, which for reporting looks like the team isn’t asking and for the team doesn’t give them a clear idea as to how to respond to the patient’s needs.
    • Additionally, the transportation questions still need to be broken down even further. There isn’t a way to say “yes” “no” and “choose not to respond” for both
    • Answer: The current screener does have each question broken out separately.
  • Is there any plan to have the PRAPARE template talk to other templates within the EHR...for example, I've gotten a few requests from health centers in the last few months, to have medical information from PRAPARE copy to the Social History template for those items that are answered on both templates? 
    • Please send Joy needs particular areas/examples from the other templates in NG
  • How do I find what version of NG our CHC is on?
    • Include directions
  • Where is the best place on NG Success Community to find PRAPARE training and resources including codes and such? 
    • Include details as well as how to submit “Ideas” for improvement efforts 
  • For FPL, can it be automatically populated? Most ptʻs do not know what their FPL is and we donʻt want the MAʻs or providers wasting time going into demographics to look for it on demographics. Or if it canʻt be auto populated on the template can it look like the veteran status question that is in parenthesis?
    • Yes, in the upcoming upgrade in Spring 2022.
  • Can we get a tipsheet on what CPT codes are mapped for the PRAPARE answers? 
    • Yes… it matches with the codes from PRAPARE. 
    • Yuriko will send Joy the latest version of Data Documentation so she can share with NG tech staff.  
  • Can the tool be loaded into Clearwave as it is patient centric and would be helpful if this can auto populate the temple?
    • June Lynch: form at the kiosk for check-in. It goes into PDF and then into “categories”. Someone does need to complete the PRAPARE. However, Clearwave is working on it.  
  • What do we do if the information on the UDS page is incorrect and we notice it when we are trying to do a PRAPARE, especially if it has pre-populated into the template?
    • It can be corrected in the current version of NextGen.
  • PRAPARE cannot be uploaded to the portal can it? 
    • Depends on the version of NG. Webform available to be sent via the portal. Goes into the template.
  • In the notes section, can we increase the text limit from 100 characters?
    • The limitation is to restrict problems that occur when there are too many characters in a text box. This impacts reporting.
  • Can we break down these crystal reports by provider? 
    • Crystal reports were built for later versions of NG, so there may not be full information. Not right now but in the works. 
    • NextGen Pop Health tool has a PRAPARE template built into it.
  • I've heard that NextGen will be interfacing/working with UniteUs to create/set up an interface... Is this true and if so, when is that slated to occur? 
    • API will allow for interoperability. NG is stabilizing the current platform so that it can allow for additional APIs. Joy will ask if UniteUs is on the docket. 

Comments/suggestions

  • This is a suggestion, I would love to see areas where the person completing the PRAPARE with the patient can make comments. For example, being able to explain a person's housing situation or to be able to explain what community food resources a patient may be utilizing or has utilized. As of now, we have to open a separate template to document any elaboration on a patient's responses to the template. I think this is helpful when talking about referrals to community resources.
  • Davida Carr - To piggy back on that…out here in AZ, we’re scheduled to have an HIE-style portal for SDOH  in the fall. As providers we’d need a way to document that we’ve accessed the NowPow portal.
