What is working well with using the PRAPARE template in the EHR?

  • Formalized template is helpful. Nice to not need an encounter open in order to use the form, as it’s often completed outside of a patient visit.
  • Able to pull reports from structured data.
  • No longer using PRAPARE in EHR. Was too hard to pull data/reports. Using a referral platform vua CHWs currently w PRAPARE
  • it's not.  We built out structured question because we needed to modify questions due to our patient population
  • Not using it - had to build our own structured fields which helped with reporting
  • Finding patients that need to be referred to our Patient Resource Coordinator for additional support.
  • Staff are able to complete  the PRAPARE in the patient chart during the encounter, whether in person, telehealth, or telephone
  • In-person assessment with staff completing the template.
  • Just in the implementation stage



What challenges have you had using the PRAPARE template? 

  • Not able to document answers for all elements. Specifically material security and transportation. Can’t say “yes” “no” and “choose not to respond” for all elements. End up looking blank which messes with reporting, care team support, and payer feed.

  • Not able to modify or add items specific to our patient population.
  • I think most of the challenges is training the people who use it how to use it.

  • Making sure that it is mapped to mark all measures needed.  We collab with Azara also.
  • "Are you referring to the Prapare smart form as the template?  our staff says it doesn't ask the questions that need to be asked and it cannot not be edited.
  • is there a way that patient's can enter the information w/o having go through the portal"
  • There was a glitch that stopped allowing the patient demographics from populating into the form, which caused an issue because all PRAPARE questions weren't being answered. Also, there is no reporting that comes with the EHR so we had to create it.

  • Not  inclusive of all the needs of patients

What changes would you like to see in the EHR PRAPARE Template? 

  • Mapping and reporting on referral resources.   Structured data for reporting
  • Allow modifications or add structured data builds in EBO reports
  • Standard reporting for the PRAPARE. It doesn't make sense to pay for the template to be added and not receive reporting capabilities to analyze the data from the template
  • Trigger ICD-10 codes and cpt codes for services provided, triggering the referral when it is checked

Where do you find that PRAPARE is not enough in getting the full picture of patient-level SDOH needs?

  • Our biggest problem is not having the resources to assist the patients with some of their needs
