Questions to Ask

  • Is the Vendor Information Form going to be online such that the vendor fills it out themselves?

    • Can we assume that the fields from the Vendor Information Form will not be separate/will be transferred to the registration form (not asking the vendor to

  • In Cobblestone, can accounts from the same organization / company be linked in some way - is there an opportunity for multiple accounts to work on the same information form and/or RFP?

  • In Cobblestone, is it possible to export in CVS/Excel- like to be able to sort responses - including the identifying information that was included in the VIF

  • Is HCOG still using their QVL?

Questions to Include on the Registration form

Guiding principle: Most things that are scored or evaluated go on the RFP would not be included on the Registration Form.

  • Questions for Safety Net Partners

    • Are you an CHC, HCCN, PCA, Other

    • What HCCN, PCA, other network are you part of?

    • What EHR(s) do you use? (Maybe one per Q, option to add another as relevant)

      • Where is it hosted?

    • Do you use a population health program?

    • Topic areas (e.g. blood pressure, cholesterol, women’s health)

    • Skills to learn or share

      • [JD: I envision this leading to targeted notifications re: learning communities OR expert speakers]

  • Questions for More General

    • Checkboxes for different skill/interest areas

    • Keep general information (e.g., organization, name, etc.) if we choose to export as a spreadsheet

      • Will reduce reviewer burden during the review process

  • When they apply for RFP, first step might be to verify that info is correct, and edit if it is not
