  1. Engage three Community Health  Centers, establish project goals, regular meetings
  2. Develop two personas highlighting real postpartum patients (MEC/SPR and GDM)
  3. Baseline clinical and data workflows and updated eCQM specifications and monthly updates
  4. Map clinical workflow and dataflows and identify areas of improvement and create an implementation plan for proposed improvements  
  5. Conduct baseline data pull using Data Dictionary and Data Pull Instructions provided by NACHC including evaluation and feasibility of updated eCQM specifications 
  6. Apply iterative quality improvement strategies to close post-partum care gaps in contraception and post-GDM screening and counseling
  7. Document changes made to clinical workflows, staff training materials, lessons learned, challenges and best practices

This project was three different projects through Y02.  Beginning in Y03 (August 1, 2020) it is one project consisting of: 867 Clinical Decision Support (CDS) MEC-SPR Guidelines, 866 Post-partum Gestational Diabetes Mellitus (GDM) screening and (project number ) (eCQM) post-partum contraception and Post-partum eCQMs.

The UCSF project (854) is separately funded (non-CDC) and has a different funding period but is related to the women's health projects.
