CHC Lubbock: Lindsay Grace (NP, works with PCMH and UDS measures), Jordan Dukes (Project Coordinator), Veronica Ramirez (EHR specialist, EPIC), Dr. Allan Camacho (Medical Director),

NACHC: Jonathan Uy

CGH: Rocio Payne, Nicole Kemper

Dr. Camacho said they want to improve their internal processes for Adult Immunizations

This team was on PICC program. Same team? Yes, per Allan, but not everyone on this call was on that team.

  • Work on PICC program: worked with 3-4 health centers in Lubbock to improve covid vaccines. Now on EPIC, went live in Nov 2022. Was on Intergry previously (Greenway)

  • Hard to reconcile immunizations without direct contact with other agencies. Want to do bidirectional interface to better have information on immunizations.

SIPS Program Interest:

  • Initial discussions include wanting to focus on one of their offsite clinic; have 1 provider (NP), pneumo vax. Care gaps are in place in Epic. Have been working with HCN.

  • Local pharmacy use InTrak to report immunizations, wanted to ask what Walgreens, CVS, Walmart, others use for code mapping. Hard for the CHC to call them to get those records and hard to get the info from patients. They use Care Everywhere. “Outside source” is listed on patient vax record in Epic, but can’t see which site exactly by name.

  • Action Item: @Rocio Payne Rocio to email Karen Iversen and copy Allan Camacho asking if HCN knows CHC Lubbock is participating in the SIPS cohort.

Jonathan reviewed the data requirements for the project.
