PROGRESS SINCE LAST WEEK


Emory focused on creating a framework that took the curriculum and made some modifications and recommendations. (Link)

Framework creates a check-in for coaches to help them check for systemic racism, social determinants of health, bias, cultural humility and considerations for people with diabetes. Framework also reviews language.

Considerations for people with diabetes - Framework takes into consideration the role of the coach vs the role of the diabetes care teams.


ACTION ITEMS FOR THIS UPCOMING WEEK


  • The team will provide feedback on Emory's framework.
  • - Emory will write the introduction of the document
  • Emory will start implementing the framework on the 16-weeks file from CDC. 
  • Emory to add biases for virtual consideration/distance delivery to the framework.
      • Considerations should be based on evidence-based resources, instead of google research (maybe add on the first session)
  • Each consideration should have a definition for each, and a framework that supports the utilization of those buckets. Utilize CDC Equity Framework
  • Lay out the requirements for the patients with diabetes, and what needs to be done before the NDPP sessions
  • Camila Silva& Luke Ertle (Deactivated) - Share confluence page with the entire team

AREAS OF RISK/UNCERTAINTY


Curriculum:

How to make sure that under SDOH: build space for social, economical, & political considerations for stress management. As well as building a space for people to share personal challenges. 

For coaches: There should be a space for these issues, as they arise in individual groups, are examples of things that the regular 'coach connect' calls will address. Lifestyle Coaches will need support for these issues.

How will the focus on carbs will interface with the curriculum?

Please note, a change on the RFP is the PCAs/HCCNs can apply together. 

One concern from Marcy: It has taken 1-3 months for health centers to recruiting patients. So the timeline should be longer for recruitment and preparation.

Concern also around the scales, if they have Bluetooth capabilities or not, since that will be a time consideration.
