  • What is one thing you learned during this project year?

  • DCPCA - rural vs urban still have similar barriers, call about engagement was very helpful - not alone; public perception of what health center is - people not always excited to seek healthcare so have to persevere and educate - need to step into empathy and creativity to engage patients

  • WY - trust is really important with patient population and if whole team is sending the same message and promoting the same programs and education will help the patient to positive health outcomes which also helps trust and have them be an ambassador; important that new staff are also educated to different departments and services

  • EE - from a non-clinical standpoint - have noticed that it is important to understand health literacy of the patinet - a lot of cultural, racial misconceptions that people don’t understand importance. requires communication and digging - it is a diverse problem. Taking the time to go into details about their care and self-management
