1) Identifying patients who are pregnant, pregnancy risk factors requiring follow up and scheduling post-partum visit

- Risk factors including: HTN and cardiovascular disease, GDM, mental health, substance abuse
- Operational considerations of post-partum care-- coordinating with neonatal visits, linking records and engaging pediatrics, coordinating with hospital care for follow up
2) Ensuring patients have timely post-partum care
