Submit Implementation plan to design and conduct Health Information Technology (HIT)-enabled quality improvement strategies; implement an iterative process of using data to improve the interventions to improve results for comprehensive post-partum care in the following five areas:

• Cardiovascular (blood pressure, cardiomyopathy, thromboembolic events)

• Diabetes/GDM

• Anemia

• Post-partum depression/mental health

• Substance abuse

Use data dashboard metrics and data elements to support continuous quality improvement according to Implementation plan.

One final report on project’s accomplishments, challenges and Best Practice examples identified as a result of the project, recommendations for scale and spread to other HCCNs and health centers, including those collected in Harvest Meeting.
