Convening Aim
Summarize the best practices and lessons learned from FQHCs implementing COMMIT during 5 years of CDC grant funding.
Intended Meeting Outcomes
Co-create an Implementation Guide to support FQHCs interested in offering Intensive Health Behavior and Lifestyle Treatment (IHBLT) to comply with USPSTF and AAP recommendations.
Identify unique FQHC challenges and opportunities to implementation and provide pragmatic tools to scale and spread needed programs and resources for patients.
