Convening Aim

Summarize the best practices and lessons learned from FQHCs implementing COMMIT during 5 years of CDC grant funding.

Intended Meeting Outcomes       

  • Co-create an Implementation Guide to support FQHCs interested in offering Intensive Health Behavior and Lifestyle Treatment (IHBLT) to comply with USPSTF and AAP recommendations.

  • Identify unique FQHC challenges and opportunities to implementation and provide pragmatic tools to scale and spread needed programs and resources for patients.
