Welcome & NACHC Updates

  • Scope of this year’s activities - User stories, data elements related to those user stories, data that gives us further info.

  • Data Dictionary

Ray anticipates three papers that can come from this work.

OCHIN

User story is with a different site from last 2 years.

There is a great implementation story at the first site- they have been doing a lot of training - lots of improvement over time - they are trying their best

  • the lists in Epic are still confusing - they don’t know what to choose.

AC

Different site from last 2 years - new site is in a trucking area.

Extra issue - the forms that occ med practitioner has to fill out are complex and need to be filled out in a certain way, so if the sole provider cannot be there, it’s not possible for someone to fill in.

HCN - with La Casa

CHC created a cheat sheet for the staff showing the:

business name → code → job description ->code -

Identified the codes for industries and occupations in food service, and looked up problems and meds within pool of workers to see if there are trends.

Continuing to refine how to make it easier to get correct data - experimenting with putting in patient portal so patient can fill in; also working with staff for techniques to help maintain patient privacy, reassure them.

Discussion: Data Dictionary changes based on User Stories

  • We are using ICD 10… not ICD-9

  • Time frame -

    • depends - if you have ODH data before the start of the project.

    • Data pull will be since the start of the ODH Project 2021 onwards (or earlier) but partners can choose through June 30, 2024. You don’t have to send us the data, you would send the data that proves the use case - brainstorm possible intervention, you don’t have to do it, just plan how you would address the issue. How feasible would it be to implement a solution.

how can ai help with the form-filling out issue

NIOSH Industry and Occupation Computerized Coding System (NIOCCS) their coding system.  It is AI-powered. Some students have small projects to use it.

https://csams.cdc.gov/nioccs/

  • Building in synonymns - large language models.

  • It is hard to use an AI model with free text.

  • Taxonomy is parent-child vs ontology is relationships ICD 11 is ontology.

What is the lift?

HCN - yes juice is worth the squeeze - occupatoin and employment have such a large impact on health outcomes and access to servcies. it does not get as much limelight as it should.

It gives us a unique tool to work with that others may not have - we are hoping it becomes widespread..

AC - iti s important how wfh impacts our health…

on a scale of 1-10, this is a 3-4 for difficulty in implementation. YOu are asking staff when you are short staffed to collect information. You can’t just hand our populations and ipad and say “fill this out” it is a bit more difficult than so simple… you have to talk through it, and provide support. We sit in front of computers all day and fill stuff out all day. People who don’t look at computers all day - its not intuitive for them - how do we make it so that they geel empoowered to fill out that information if the staff does nto have time - how do we give them the opportunity to capture tht information.
