2 min | Welcome & Icebreaker | Group | What key takeaway or lesson learned from the Preventing Heart Attacks and Strokes project experience would you like to share? HFP: Patience- this takes time. These are big changes to thoroughly and make thoughtful changes. CHESPenn: It is important to learn on the ground what this looks like and how the implementation effects people. FPCN: Definitely been some progress but more to do. It has been helpful to re-evaluate some of our workflows. A lot of our providers are engaged in SMBP which is a big positive. Berks CHC: It has been a huge learning process for us. Biggest takeaway. This is not something that can be done quickly and that is ok. We have been able to evaluate htn control on a large scale at Berks. I sometimes feel like I put a lot of pressure on myself to move things along- knowing that this is a PI project and it takes time and re-evaluation. |
