What key takeaway or lesson learned from the Preventing Heart Attacks and Strokes project experience would you like to share?

HFP: Patience- this takes time. These are big changes to thoroughly and make thoughtful changes.

CHESPenn: It is important to learn on the ground what this looks like and how the implementation effects people.

FPCN: Definitely been some progress but more to do. It has been helpful to re-evaluate some of our workflows. A lot of our providers are engaged in SMBP which is a big positive.

Berks CHC: It has been a huge learning process for us. Biggest takeaway. This is not something that can be done quickly and that is ok. We have been able to evaluate htn control on a large scale at Berks. I sometimes feel like I put a lot of pressure on myself to move things along- knowing that this is a PI project and it takes time and re-evaluation.

May 7, 2024: Dr. Lisa Soltani and Dr. AnnMarie Overholser will review four case studies of health centers or networks. The case studies will highlight improvements and opportunities with the data and interventions in order to approach goals by the end of the project year.
