• One thing you learned during this project year

  • Lauren - learned from other organizations, ex. text messaging campaign idea from Cincinnati. Now doing outreach to patients on no therapy and monotherapy

  • Stephanie - comparing data from who opened text messages to patient gap list to see effectiveness, develop SOP for interventions

  • OCHIN - meetings with other networks/health centers to share information, ideas, data, alternating Town Hall Call with Check-In Call is working, space to think outside of the box and test/share what is working vs not working

  • Hallie/CHCN - sharing challenges just as often as the wins, being open to discussing challenges and discussing solutions

  • Trushna - connection between care team nurse and patient is important, can be a bridge between provider and patient and shift the culture, outreach to MA and providers is important

  • Laura - efforts show that more than just clinical staff are needed to make a difference, give a case to leadership to dedicate staff to these efforts, shown value to continue these efforts and expand to addressing SDOH, appreciate sharing ideas with other health centers
