- NACHC and AAPCHO would like to work with folks on the ground level who are collecting data to:
- Get a better understanding on how to best enter data
- Know who is responsible for entering the data/information
- What action is taken once the data is gathered and how we can streamline/advance this aspect to make things easier in order to address the patient needs in real time
- Understand how PRAPARE is operationalized in the field and how we can work with respective vendors to improve the process

- If you have colleagues who are interested in joining the PRAPARE Tiger Team you may share the confluence page link or meeting registration link. Prior to each meeting there will be a roll call to understand who is in attendance.
- What is the normal process for implementing the PRAPARE Form?
- Refence the Implementation Toolkit for more information on how to implement PRAPARE Chapter 3: Strategize the Implementation Process
- Encouraged to have the latest version of the PRAPARE tool available within your EHRs.
- How will this work complement the Gravity Project?
- Gravity Project is a board ranging multi-disciplinary effort- creating a taxonomy of all items that would live in the SDOH space which different from PRAPARE. PRAPARE is designed to be functional and easily implemented at the local level.
- Will any of the core PRAPARE questions be modified due to COVID and the economy fall of 2020?
- We are aware that PRAPARE does need to evolve and we are hoping the Tiger Team is a way to receive effective feedback. NACHC is in the early stages of developing "PRAPARE 2.0":
- Reviewing if the right risk factors are being captured so that health centers may deliver the care their community need
- Focus on a way to better document the interventions when a positive screening occurs
- We are aware that PRAPARE does need to evolve and we are hoping the Tiger Team is a way to receive effective feedback. NACHC is in the early stages of developing "PRAPARE 2.0":
- Would this group work with additional EHRs to improve the PRAPARE form, data reporting and coding ?
- Yes- goal is to engage vendors so that we may understand how we can improve usability of the tool and how to expand the functionality. First step is to understand the current workflow. how you use and record your data and partners
- Where can you find the screenshots for the EHR templates?
- Through this group we hope to have the opportunity to update the EHR template screenshots
