HC History Notes:

  • PrimeCare: history based in a residence program...helps build the pipeline

  • Evara: started small, expanding regularly

  • Settlement: Harlem, active since 1977, lots of immigrant population

  • Family Christian Health Center: started with 1 with a group of 4 family practictioners (founder is now CEO), "today care"

  • Heartland: started as demonstration project for homeless 30+ years ago. Strength based, trauma informed, harm reduction approach. Comprehensive services

  • Neighborhood FP: 40+ years, refugee population, looking at ethnicity/race is key for this work due to assortment of pop

  • Premier: since 1979, started as Migrant Health Center (still carry this), though pop has trended down due to farms reducing, homeless, veterans, disabilities, wanted to find other ways to continue to get pts vaccinated among all the barriers that exist
