NACHC Collaboration with AAPCHO

NACHC has offered to help with validation. Primary and secondary outcomes are not clear.

Heartland

  • Screening tool was implemented in the EHR a few years ago. Connected with Siouxland about risk scoring and risk stratification per NACHCs model.

  • Was Heartland apart of the team that developed the tool? No. Siouxland and Wainaie Coast were involved.

  • How many patients data were used to fit to the model? Proportional to the number of screenings - a little over 1000.

  • Utilization data components? Heartland is relying on these and created the infrastructure to capture this data manually. They should have ER visits - old school faxing, direct messages.

  • The NextGen content transferable? Not really, only transferrable to health centers within the network.

Next Steps

What is possible in terms of validation?

Would this work be easier at the HIE level? Cynchealth would have diagnoses, hospitalization and ER visits, not sure about SDOH data

Heartland would be interested in participating in validation but PRAPARE 2.0 may present some barriers.
