• Previously on CCI project: a lot of digging into community, everyone needs more information. Created roadmap, redoing websites for vaccines and doing more of a blog. Working with local health department on how to provide more education to community

  • Figuring out how to allow more walk ins

• 40,000 patients, 10 brick and mortar clinics but 25 including mobile and school clinics

  • Target this one clinic out of 6th street clinic, where the team is based and then will expand from there

  • Use Athena EHR system

  • Pneumo and TDAP or Hep B

• Data:

  • Lindsey should be doing most data

  • Concerned about trying to manually do the “small” data and having to manually review patients for pneumo eligibility

    • We will reach out to John on Informatics

On call: Peninsula: Sherry, Alder Moody

NACHC: John Gresh, Rocio Payne

Sherry was having technical difficulties connecting from Mexico and had to drop off call. Rocio screenshared the chart audits for discussion, but Alder said Lorie and Lindsay did the chart audits and would have the answers to our chart audit questions. Sherry did the mini intevention. Rocio will send an email to follow up with the team on these items since they were not on the call. The call lasted only 10 minutes.
