Quality manager Joyvell uses population health software to gather data for Crinela Todea patients (vaccination status for COVID and pneumococcal). Mariella RN performs chart reviews for missed vaccination opportunities.

Current plan is to increase vaccination rates in one provider at one site, do PDSA, then add second provider at another site, PDSA one week after.
