2.0 | Task 2 Build care continuum dashboard and provide decision support to users towards identify and closing care gaps. Create and execute an implementation plan for the care continuum cascade developed as part of Task 1. Test and deploy the dashboard in a pilot at one or more health centers. Engage in troubleshooting to understand measures and outcomes. Discuss challenges and successes of the approach taken. Gather data on patients, users and outcomes of the dashboard and its related CDS tools. Document proposed scale and spread of the tools developed to support HCV care.
