Deliverable 1: Sprint One -- Establish Baseline Data

Establish baseline data for adult influenza and COVID-19  immunization using a Human Centered Design approach and targeted interventions by completing and submitting the Data and Documentation Baseline Questionnaire tool to determine each participants’ focus area, specific interventions (products and activities), and assigned staff.

• Attend Sprint 1 kick-off meeting with NACHC due 08 Apr 2022
• Complete and submit the Data and Documentation Baseline Questionnaire
• Based on Data and Documentation Baseline Questionnaire, coordinate with NACHC to identify focus area, staffing, and tasks/products for Sprint 1

• Data and Information

• Establish influenza and COVID-19 vaccination rate improvement goal (minimum of 10 percent improvement should be targeted)
• Submit an implementation plan to achieve improvement goal
• Conduct and submit a baseline data pull and assessment of data quality
• Submit one persona

Deliverable 2: Sprint One: Implement and Monitor Strategy

Initiate the Sprint 1 implementation plan, conduct ongoing monitoring of progress, and share experiences with other health centers, HCCNs, and PCAs. NACHC will facilitate the design of focused sessions for health centers that share similar populations, geographic regions, or other similarities that warrant collaboration across sites.

• Administrative

• Meet monthly with NACHC to review strategy and progress against project goal(s).

• Participate in scheduled Learning Community session(s)

• Share lessons learned, facilitators, barriers, and best practices with other health centers, HCCNs, and PCAs during all-participant meetings.

• Data and Information

• Provide monthly data extracts using NACHC’s data dictionary and data pull instructions.

• Quality Improvement Strategy

• Implement iterative process of using data to drive improvement of adult immunizations.

Products:

• Monthly NACHC/Health Center meetings

• All-participant meetings, to present best practices on adult immunizations that can scale and spread

• Summary of lessons learned, challenges, and best practices

• Monthly data extracts of data compared to the baseline to NACHC

Deliverable 3: Sprint 2-Establish Baseline Data

Establish baseline data for the selected immunization using a Human Centered Design approach and targeted interventions by completing and submitting the Data and Documentation Baseline Questionnaire tool to determine each participants’ focus area, specific interventions (products and activities), and assigned staff.

• Establish selected immunization vaccination rate improvement goal (minimum of 10 percent improvement should be targeted)

• Submit an implementation plan to achieve improvement goal

• Conduct and submit a baseline data pull and assessment of data quality

• Submit one persona that identifies the challenges to improving the selected immunization’s vaccination rates, from the patient perspective of navigating personal, environmental, and/or health care-related barriers to vaccination (relevant personas from prior years may be updated and resubmitted)
