Guest speakers from Azara, Missouri Primary Care Association, and Michigan Primary Care Association.

Learning Objectives include:

  • Explore the reporting features within population health management tools to create dashboards, registries, and reports to act on SDOH data that is collected using PRAPARE. (i.e., we will use DRVS from Azara, but the content is relevant to everyone)
  • Hear how PRAPARE data in PH dashboard has been leveraged to inform population health efforts at health centers and drive SDOH statewide initiatives.
