Administer/Refer Assignments Due May 30

  • Administer/Refer Activity: May use the Word Doc template for gathering responses, but please submit your responses via this Microsoft Form Link

  • Complete a 10-chart audit for gaps where the Administer/Refer standard would be applied

  • Identify a mini-intervention to apply for 1 - 2 weeks to try and close the identified gaps

  • Complete a post 10-chart audit to evaluate effectiveness of the mini-intervention trialed

Please submit weekly “small” data on Confluence by May 23

Please submit “big data” (the de-identified patient-level data) on Confluence by May 30 if you have a Data Use Agreement in place with NACHC. The SIPS Data Walkthrough page is here for your reference.
