Task 1:  Project continuation and health center engagement

• Establish at least one pilot site which will advise the implementation team and use the CDS to support HIV testing and risk assessment.

• Attend project kickoff

• Complete initial technical assessment and populate project team data.

• Engage with CDS team to discuss CDS risk assessment and workflow issues and data extract.

Task 2: CDS technical implementation, testing and deployment

• Implement HIV risk assessment and test CDS tools from the FHIR Implementation Guide.

• Work with partner sites to troubleshoot and improve the implementation.

• Engage additional sites and health centers with interest in supporting HIV testing as possible expansion sites.

Deliverable 2.1 Attend and present updates (verbally or via slides as appropriate) at monthly All Partners calls

Task 3: Refinement and Data Analysis

• Provide data for evaluation of the CDS tool’s impact on the provision of HIV care.

• Provide recommendations for scale and spread of this and other related CDS tools.

Deliverable 3.1 Submit 6 months of post-implementation data.

• 6 month post-implementation data submitted.

Deliverable 3.2 Harvest Meeting attendance and slides highlighting changes in workflow, uptake challenges/successes, lessons learned, results of implementation projects.

• Harvest meeting slides (template to be provided by NACHC)
