• Lauryn Berner (Health Care for the Homeless) will join us on May 11, 2023 at 3pm for the first half hour to talk through the journey map with the core design team

• Coaching Support

  • One coach assigned 1-2 teams

  • Meets bi-monthly during alternate large session months

• Core Design Meetings

  • Continue to meet weekly for the next four months

  • Re-evaluate the meeting cadence then, consider 2x a month

  • Suggest monthly planning meeting (prior to the large sessions/coaching & mentorship sessions) and then a monthly debrief meeting

• What documents do we want to use for the learning community (such as a workbook in PowerPoint form that the teams will fill in along the way)

• Courtney and Eboni both highlighted similar materials that were used in projects led by CCI- they mentioned that they could seek permission from CCI to do something similar but specific to our project.

• We also discussed when we want to have the final materials completed and then backward plan from there. I will create a timeline structure in Asana to track our progress and deliverables.

• For the May launch call, what additional information do you want to cover, and who will be leading this call?

• Informational Webinar Content: allowable costs, this is what we are looking for [three staff sharing info openly and transparently, attending all calls, sharing data]; we want you to impact # many patients lives, identify using JEDI principles, and link them to care [BH, community services, primary care])

• The team will review the charter, appendix A (learning community design), and Appendix B (measures/evaluation questions)
• LeeAnn will create a confluence page and an outline of tasks/timelines to help guide the Learning Community Design.
• Eboni will reach out to CCI to get permission to align with the workbook they use for their LCs.
