Task 1:  Project continuation and health center engagement

• Establish at least one pilot site which will advise the implementation team and use the CDS to support HIV testing and risk assessment.

• Attend project kickoff

• Complete initial technical assessment and populate project team data.

• Engage with CDS team to discuss CDS risk assessment and workflow issues and data extract.

Deliverable 1.1 Workplan and timeline for implementation of CDS tool

Deliverable 1.2 Submit data extract for baseline

Due Date: December 15, 2022             

Amount: $20,000.

Task 2: CDS technical implementation, testing and deployment

• Implement HIV risk assessment and test CDS tools from the FHIR Implementation Guide.

• Work with partner sites to troubleshoot and improve the implementation.

• Engage additional sites and health centers with interest in supporting HIV testing as possible expansion sites.

Deliverable 2.1 Attend and present updates (verbally or via slides as appropriate) at monthly All Partners calls

Deliverable 2.2 One suggestion for a user story/best practice that NACHC can document

Deliverable 2.3 Lessons learned and summary documentation of the CDS tool implementation.
