WHPP 

August 1 - July 31 is the project year; NACHC is in 4th year.  There is no current funding for Year 5.  NACHC is exploring a larger project that would go beyond the CDC funded initiative.

How do we identify patients who are pregnant? high risk?

What action plans?

How to implement action plans?

What are the care gaps? 



CMQCC 

Invited to talk about inpatient work and outpatient measures they have been working on or conditions they have been reviewing.

In addition to CA, CMQCC IL, NY, TX (one more?)

  • Sharp decline in maternal mortality in CA (stats 17 to 5?)
  • C-Section rate decline from 26 to 22% - baby outcomes have improved concomitantly
  • Improving care at scale! To change things, you have to measure them!
  • Developed a series of measures now used by Joint Commission for hospital-based care

Missing

  • Link to key support services
  • Immediate contact between hospital and clinic or office on what happened during labor and delivery

Also working on pre-partum care for prevention with March of Dimes, another on anemia among Black women.

Opportunity - how do we connect the systems, i.e., home visiting, WIC.

Next Steps

CMQCC - has been asked to provide guidelines and measures in this space 

We can identify a comprehensive approach this year and set placeholders and identify priorities, and start with those.

OCHIN was included PP depression screening and each partner is setting priorities.

NACHC will hold a design meeting in January to talk about what we all want to do and what is feasible

  • Andrea Price (Deactivated) will send a note re what you'd like to include - we have 1/22 on the calendar already - could make it 1.5 hours.  Ask Ray to contribute terminology content for that meeting.

NACHC is no longer on contract to conduct the project after 12/31/2021.  UCSF intends to execute the research plan and would like to contract with the partners.
