Tasks/Activities

• Perform or complete an environmental scan of quality measures, guidelines and evidence related to pre-, peri- and post-partum outpatient care activities in the scope of an FQHC’s practice. Summarize the findings into a review of the state of the evidence.

• Attend internal management and strategy meetings with NACHC biweekly to monthly.

• Attend relevant content and design meetings with partners (selected All Partners calls and design meetings).

• Work with HCCN partners, NACHC and CDC to prioritize the existing recommendations by clinical importance and feasibility for incorporation into a summary guidance document for CHCs providing primary obstetric care.

Deliverable 2: Quality measure and clinical decision support expertise, development and testing.

Tasks/Activities

2.1 Perform a harmonization exercise across existing guidelines and quality measure in outpatient maternal quality and outcomes.

2.2. Suggest specific existing quality measures that could be implemented and measures as yet undeveloped or in development that should be prioritized for outpatient maternal quality improvement.

2.3 Provide a document with recommendations in regard to next steps for a coherent set of measures and a guide that supports a comprehensive approach to outpatient maternal quality improvement.

2.4 Work with NACHC to support sites in California to explore and outline the feasibility and all the steps necessary to establish data connections to the state maternal outcomes database.
