Deliverable 1: Sprint One -- Establish Baseline Data

Establish baseline data for adult influenza and COVID-19  immunization using a Human Centered Design approach and targeted interventions by completing and submitting the Data and Documentation Baseline Questionnaire tool to determine each participants’ focus area, specific interventions (products and activities), and assigned staff.

Deliverable 2: Sprint One: Implement and Monitor Strategy

Initiate the Sprint 1 implementation plan, conduct ongoing monitoring of progress, and share experiences with other health centers, HCCNs, and PCAs. NACHC will facilitate the design of focused sessions for health centers that share similar populations, geographic regions, or other similarities that warrant collaboration across sites.

  •  Provide monthly data extracts using NACHC’s data dictionary and data pull instructions.
  • Quality Improvement Strategy

  •  Implement iterative process of using data to drive improvement of adult immunizations.
